This collection of evidence briefs synthesizes lessons from across the Canadian health system on the design, implementation, and impact of primary care innovations. Drawing on the Aggarwal-Hutchison Framework, the briefs span interventions such as team-based care, remuneration and incentive reform, patient and community engagement, integrated care models, digital enablement, and system governance.
